Your full legal last name.

A number where we can reach you for urgent questions about your application.
Why we need this information
These details are used only to process your incorporation file. They are never shared with third parties and are used to:
- Confirm your identity as the applicant
- Send final incorporation documents (articles of incorporation, certificate, etc.)
- Contact you if additional information is required for filing with the Registraire des entreprises du Québec (REQ)
